Why are there no "nice" restaurants (table service) that have play structures for the kids? Liability?

Probably because restaurants like that expect that any children who do come will sit quietly and behave themselves. If you want a nice quiet meal in a place like that either leave your kids with babysitter or teach them how to behave in a restaurant.
