My condom slipped off. is she pregnant?

so i was having and she was on top of me. i was going to *** earlier while we were doing it so i pulled out and squeezed the tip of my to supress the ejaculation although a little bit slipped out but i stayed hard and it wasnt a full ejaculation. i didnt think it was much so i kept going. then later when she was on top of me i pulled out and the condom was 3/4 off because i got kind of soft at the end and the inside of the condom was lubricated. so 2 questions if u answer them good enough u get 10 points. 1. do you think semen slipped out of the bottom and went up in her and if so should i get her morning after pill? and 2.how old do u have to be to get plan B from pharmacy
